In response to an exposé by the Chicago Tribune and ProPublica, the Illinois legislature passed Public Act 102-033923. It updated the Administrative Code in April of 2022 to address the use of time-out and restraint in Illinois schools (IAC 1.285). Within the code, there is a requirement to report to ISBE any use of time-out, isolated time-out, and restraint. The Physical Restraint and Time-out Form, often referred to as Form 11-01, is required to report the information to ISBE.

Forms are submitted lacking significant detail in the descriptions or questions are entirely skipped. Also, without accurate data you cannot have careful reporting or make good decisions about how an intervention is working. This module was designed to help staff complete the ISBE Form 11-01 correctly.

• Recognize the correct information required to complete Form 11-01
